In accordance with national and international laws and guidelines certain strains are designated as producers of hallucinogenic toxins with a considerable risk for human health. In order to be able to dispatch such strains the Collection Centre is obliged to check if the receiving laboratory is aware of this risks and has the potential to deal with such strains. Your order (order number) of (datum) contains such strain(s). Before the Collection Centre can sent the following strains, this form has to be completed, signed and returned to the Collection Centre by the director of the institute or his representative.

I testify that the investigators concerned are qualified through education and training to work with such material. I hereby assume all risk and responsibility in connection with the receipt, storage, handling and use of the material. It will not be distributed to third parties"
